Qualifications

• Active, unrestricted RN license in the state where Cityblock operates.
• 3+ years of clinical experience with adults/geriatrics with co-occurring chronic medical and behavioral health conditions.
• Willingness to travel for home-based member visits and familiarity with local healthcare systems.

Responsabilités

• Onboard members from the engagement and care team; clearly communicate program goals and expected duration.
• Perform screeners (PHQ-9, GAD-7, AUDIT, DAST-10) to identify behavioral health needs.
• Conduct in-person clinical examinations and collaborate to assign program intensity.
• Lead case conferences and actively participate in discussions.
• Develop care plans with members and address social needs with the Community Health Partner.
