Cityblock Health is seeking an RN Care Manager to support a panel of members with chronic conditions and behavioral health needs. You will collaborate with the care team to develop and monitor individualized care plans, conduct home visits, and coordinate with providers and community resources.

You will perform screenings, manage medications, and ensure accurate documentation while addressing social determinants of health.

Qualifikationen

• Active, unrestricted RN license in the state of employment.
• 3+ years of clinical services for adults/geriatrics with co-occurring chronic medical and behavioral health conditions.
• Willingness to travel for home-based member visits and to work with local healthcare systems.
• Experience in documentation, care planning, care coordination, and strong writing skills.
• Experience in an interdisciplinary care team with community health workers and coordination staff.
• Training or experience in wound assessment, venipuncture, and phlebotomy; care plan reinforcement.

Aufgaben

• Receive members from the engagement and care team, communicating program expectations, goals and duration.
• Conduct screening tools (PHQ-9, GAD-7, AUDIT, DAST-10) to identify behavioral health needs.
• Perform in-person clinical examinations and place members in programs of varying intensity.
• Lead case conferences and participate in team discussions as needed.
• Develop care plans with members and address social needs with the Community Health Partner.
• Conduct regular clinical visits and follow-ups per program guidelines; address needs promptly.
• Collaborate with the care team to support a panel of members in health maintenance and chronic disease management.
• Perform medication reconciliation, administration, adherence support, and education.
• Ensure chart documentation and coding (ICD/CPT) to validate gap closures; use EHRs and scheduling tools.
• Support members in achieving plan goals through coordinated care.
