Job summary

Cityblock Health is seeking an enthusiastic Registered Nurse Care Manager (RNCM) to manage a panel of rising and high-intensity members, coordinating integrated chronic disease and behavioral health care in Wilmington, NC. You will collaborate with members to develop care plans and monitor progress, reassessing needs and partnering with the Community Health Partner to build trust.

Qualifications

• Active, unrestricted RN license in the state of employment (Cityblock)
• 3+ years of clinical services experience for adults/geriatrics with co-occurring chronic medical and behavioral health conditions
• Willingness to travel for home-based member visits within the community
• Experience in documentation, care planning, and care coordination
• Experience with interdisciplinary care teams and community health workers
• Willingness to train in wound assessment, phlebotomy, and care plan reinforcement for chronic conditions

Responsibilities

• Receive members from the care team and communicate program expectations, duration, and goals
• Complete screening (PHQ-9, GAD-7, AUDIT, DAST-10) during assessment and intake
• Conduct in-person clinical exams and help determine placement in programs of varying intensity
• Prepare for and participate in case conferences, leading discussions when needed
• Develop care plans with members and address social needs with Community Health Partner
• Conduct regular clinical visits and follow-ups per program guidelines
• Collaborate with care team to support a panel of members in health maintenance and chronic disease management
• Perform medication reconciliation, administration, and education
• Address quality gaps and ensure thorough chart documentation and coding
• Use EMR, scheduling platforms to document interactions and track tasks
• Support members in achieving care plan goals through coordinated care efforts
