Cityblock Health in Winston-Salem, NC is seeking an RN Case Manager to supervise a panel of members with chronic medical and behavioral health needs. You will collaborate with the care team to create and update individualized care plans, reassess progress, and advocate for members within the community.

Responsibilities include conducting intake screenings (PHQ-9, GAD-7, AUDIT, DAST-10), leading case conferences, coordinating with providers and the Community Health Partner, performing medication

Competenze

• Active, unrestricted RN license in the state of employment.
• 3+ years of clinical experience with adults/geriatric patients with co-occurring chronic medical and behavioral health conditions.
• Willingness to travel for home-based member visits.
• Experience in documentation, care planning, and care coordination.
• Experience in interdisciplinary care teams with community health workers.
• Training or experience in wound care, venipuncture, and chronic disease care.

• Handover members from the engagement team with clear program goals.
• Perform screenings (PHQ-9, GAD-7, AUDIT, DAST-10) during intake.
• Conduct in-person exams and place members in programs by intensity.
• Lead case conferences and develop collaborative care plans.

• Conduct regular visits and follow-ups per program guidelines.

• Perform medication reconciliation, administration, and education.

• Assist members in achieving their care plan goals.
