Job summary

Cityblock Health is seeking an RN Care Manager to oversee a panel of members with chronic medical and behavioral health needs. You will collaborate on care plans, coordinate with providers, and conduct regular clinical visits to support health maintenance and disease management.

Travel within the community for home-based visits is expected. A valid RN license and 3+ years of relevant experience are required; you will work with an interdisciplinary team and participate in care coordination

Qualifications

• Active, unrestricted Registered Nurse license in the state you are seeking employment.
• 3+ years of experience providing clinical services to adults and/or geriatric individuals with co-occurring chronic medical and behavioral health conditions.
• Familiarity with travel for home-based member visits within the community.
• Experience with documentation, care planning, and care coordination; excellent writing skills.
• Experience in interdisciplinary care teams and collaboration with community health workers and care coordination members.
• Prior experience or willingness to train in wound assessment, blood draw (venipuncture & phlebotomy), and care plan reinforcement for chronic conditions.

Responsibilities

• Receive members from the engagement and care team and communicate program expectations, goals, and duration.
• Perform screenings (PHQ-9, GAD-7, AUDIT, DAST-10) to identify behavioral health needs.
• Conduct in-person clinical examinations and determine placement in programs of varying intensity.
• Lead case conferences and participate actively in discussions.
• Develop care plans with members and address social needs with the Community Health Partner.
