Cityblock Health seeks an RN Care Manager to oversee a panel of clinically complex members, coordinating with the Cityblock Care Team and community partners to support impactful clinical programs and pathway graduation.

The role blends field visits with remote work, performing assessments with tool-based screeners like PHQ-9, GAD-7, AUDIT, and DAST-10, and leading care plan development and case conferences.

Qualifications

• Graduate of an accredited school of nursing (RN).
• 3+ years of experience in clinical care management.
• Strong problem solving and clinical judgment with collaboration.
• Proficient in communicating across remote and in-person settings.

Responsibilities

• Receives members and communicates program expectations, duration, and goals.
• Performs self-efficacy and condition screeners including PHQ-9, GAD-7, AUDIT, DAST-10.
• Conducts in-person exams and places members in programs of varying intensity.
• Leads case conferences and develops member care plans with social support partners.
• Monitors care plan progress and coordinates with external providers and community resources.
• Performs medication reconciliation, administration, and education.
• Documents thoroughly to close quality gaps and uses EHR tools for data capture.
