Job summary

Aura Health in Albuquerque is seeking a Registered Nurse Case Manager to support inpatient, outpatient, and utilization management services within a care management department. You will coordinate care, discharge planning, and transitions across age groups, collaborate with a multidisciplinary team, and support readmission reduction through referrals and transition of care.

Qualifications

• Graduate of an accredited nursing program with at least 1 year of relevant experience.
• Active RN license in the state of practice and CPR/BLS certification.
• Bachelor's degree preferred; bilingual skills (English/Spanish or other languages) are a plus.

Responsibilities

• Provide care coordination, discharge planning, and safe patient transitions across all age groups.
• Participate in multidisciplinary rounds and collaborate with providers, therapists, pharmacists, and social workers.
• Support outpatient care through referral management and home health coordination to reduce readmissions.
• Conduct utilization reviews and work with payors on billing and denials.
