• ADN required; BSN preferred or to be obtained within 5 years.
• 2 years clinical nursing experience with utilization review or case management desirable.
• National Case Management certification preferred.

Responsibilities

• Manage a designated caseload across the continuum of care.
• Coordinate utilization management and status determination.
• Lead discharge planning and care transitions.
• Collaborate with interdisciplinary teams during rounds and conferences.
• Maintain accurate and timely documentation and support performance improvement.
