Job summary

UNM Hospital in Albuquerque seeks an experienced nurse manager in Care Management Services to lead patient care coordination, triage, and multidisciplinary collaboration. You will supervise staff, ensure efficient assignment of case loads, and oversee discharge planning to optimize patient outcomes.

The role requires RN licensure in NM or reciprocal access, 3 years related experience, and a Bachelor’s Degree (Master’s preferred). Sign-on and relocation assistance may be available.

Qualifications

• 3 years directly related experience
• RN license in NM or per reciprocity
• Bachelor's Degree required
• CPR for Health Care/Resuscitation required

Responsibilities

• Identify and triage patients for case management interventions
• Supervise staff and coordinate workloads and scheduling
• Coordinate services across departments and community resources
• Perform data collection and analysis for patient care plans
• Develop and monitor multidisciplinary care plans and discharge planning
• Lead meetings and facilitate care conferences with care team
• Ensure adherence to hospital policies and patient safety protocols
• Provide education to patients and families about care plans
