Job summary

University of New Mexico Hospital – Albuquerque is seeking an RN Case Manager for the Care Management Department. The role includes inpatient and outpatient care coordination, discharge planning, and utilization management.

You will work with a multidisciplinary team, attend rounds, and help navigate referrals and care transitions. Weekend on-call and some holiday coverage are required as part of the position.

Qualifications

• Active RN license in New Mexico or eligible for licensure.
• 1 year directly related RN case management experience.
• CPR/First Aid certification (BLS) required or must obtain within 30 days of hire.
• Nursing program graduate with nationally accredited program.

Responsibilities

• Coordinate care and discharge planning for inpatient stays.
• Attend daily multidisciplinary rounds with the care team.
• Support outpatient care management and referrals to DME, home health, hospice, etc.
• Participate in utilization management activities and chart reviews.
