AdventHealth in Winter Garden, FL is seeking a Care Management leader to oversee daily operations in inpatient, observation, and emergency settings. The role focuses on discharge planning, documenting choices, and regulatory compliance to ensure timely, quality patient care.

You will lead care managers, participate in daily rounds, coordinate through hospital leadership, and contribute to throughput improvements.

Qualifikationen

• Bachelor’s degree required; Master’s preferred.
• 1+ leadership experience in care management required.
• Care management experience required.
• Accredited Case Manager (ACM) or CCM preferred.
• Formal de-escalation training within 90 days.

Aufgaben

• Oversees the daily operations of the Care Management department in inpatient, observation, and emergency department settings.
• Ensures documentation and compliance for discharge planning, patient choice, and other regulatory requirements.
• Ensures staff participation in daily multidisciplinary rounds and actively participates to optimize the interdisciplinary care coordination.
• Monitors staff documentation, reports, and dashboards to ensure adherence to the company-wide standard processes and quality care.
• Oversees care manager's providing the full discharge planning from admission to discharge, assuring high quality and timely care management interventions to avoid delays and unnecessary readmissions.
• Collaborates with hospital leadership and executives to elevate pace of care delays and ensure daily progression of care and throughput.
