AdventHealth in New Smyrna Beach, FL is seeking a senior care management leader to oversee recruitment, retention, and daily operations of the Care Management department. You will drive discharge planning, care coordination, and social services while ensuring CMS CoPs compliance across the organization.

Collaborating with interdisciplinary leaders, you will lead change management, monitor metrics, manage budgets, and mentor staff to improve patient outcomes and staff engagement.

Qualifications

• Bachelor's in Nursing required.
• Master's in Nursing preferred.
• 5+ direct care management experience required.
• 3+ care management leadership experience required.
• 2+ frontline nursing experience required.
• RN required; ACM/CCM preferred.

Responsibilities

• Oversees recruitment and retention activities to ensure adequate staff and leadership in care management.
• Engages with post-acute care entities to improve patient care transitions and reduce readmissions.
• Leads technology change management to improve staff efficiencies and patient outcomes.
• Directs daily operations of care management, including discharge planning, coordination, progression of care, and social services.
• Ensures CMS CoPs for Discharge Planning compliance and participates in corporate care management meetings.
• Develops and implements care management policies, budgets, and standard operating procedures.
