Job summary

Orlando Health, Inc. in Lakeland, FL is opening a new Lakeland Highlands Hospital unit and seeks a Registered Nurse with care coordination experience to promote efficient hospital resource use from admission to discharge.

You will collaborate with clinical teams, assess patient risk factors, and support transition planning to appropriate care levels. Florida RN licensure and Handle with Care certification are required or pursued on hire.

Qualifications

• Graduate of an approved school of nursing.
• Must hold and maintain a current Florida RN license.
• Handle with Care (HWC) Certification within 90 days of hire.

Responsibilities

• Promote and facilitate effective management of hospital resources from admission to discharge.
• Collaborate with the clinical team to identify patients who benefit from care coordination services.
• Assess patients' risk factors and the need for care coordination and utilization management.
• Lead and participate in interdisciplinary patient care conferences and complex case reviews.
