Job summary

UnityPoint Health is seeking a Remote RN Care Coordinator to support safe transitions from hospital to home through telephonic care coordination and patient education for high-risk patients.

The role involves post-discharge outreach, detailed medication reconciliation, and coordination of services and community resources, with EPIC in-work queues and collaboration with providers. The position requires flexibility for weekend and holiday rotations to ensure continuity of care.

Qualifications

• Active RN license and at least 1 year of clinical experience.
• Home care or ambulatory care experience preferred; EPIC familiarity a plus.
