Job summary

IMA Medical Group is seeking an RN Care Coordinator to support Polk County patients navigating ED visits and hospital admissions. You will coordinate across hospitals, PCPs, caregivers, and care teams to ensure smooth transitions and timely follow-up.

The role includes both remote coordination and on-site hospital/clinic visits, requiring strong clinical judgment, communication, and care-management experience in a fast-paced environment.

Qualifications

• Bachelor-level nursing degree or higher.
• 3+ years of hospital and/or clinic experience.
• Active Florida RN license.
• CPR/BLS certification.
• EMR experience (Athena preferred).
• Proficiency with Microsoft Office (Teams, Outlook, Excel, Word).

Responsibilities

• Review hospital portals and HIE reports daily to identify inpatient admissions and ED visits.
• Assess risk and care needs; identify barriers to avoidable utilization or readmission.
• Perform hospital visits and follow-up during admissions and ED encounters.
• Coordinate transitions of care including discharge planning and post-acute services.
• Collaborate with hospitalists, PCPs, Care Managers, and care teams on care plans.
• Educate patients and caregivers on diagnoses, medications, discharge instructions, and resources.
• Communicate updates to PCPs and care teams; participate in care management meetings.
