Qualifications

• Graduate from an accredited school of Nursing.
• Washington Registered Nurse License at hire.
• Bachelor's Degree in nursing is preferred.
• 1 year of clinical experience is preferred.
• Experience in patient and staff education is preferred.
• Experience in conflict resolution is preferred.

Responsibilities

• Provide care transition services for hospitalized patients.
• Coordinate with physician team and discharge planning staff.
• Perform nursing assessment and education.
• Act as liaison and support for patients, families and staff.
