Job summary

Northshore Home Health Care is seeking an experienced RN Care Coordinator to assess transitional care needs and coordinate services across the care continuum. The role supports safe patient transitions from the hospital by partnering with physicians, interdisciplinary teams, patients, and families.

Ideal candidates are compassionate, collaborative, and dedicated to improving patient outcomes, with at least one year of acute care nursing experience and a current BLS certification.

Qualifications

• Active RN license required.
• At least 1 year of acute care nursing experience.
• Current BLS certification.
• Strong collaboration and communication skills.

Responsibilities

• Assess transitional care needs and coordinate services across the care continuum.
