Job summary

Northshore Home Health Care seeks an experienced RN Care Manager focused on patient assessment, care planning, and coordination. You will conduct comprehensive assessments, develop personalized care plans, address barriers to care, and facilitate communication among patients, families, and healthcare providers.

Responsibilities include monitoring patient progress, coordinating referrals and transitions of care, educating patients on self-management, and maintaining accurate health records.

Qualifications

• Bachelor’s or Master’s Degree in Nursing required.
• Minimum 2 years of clinical nursing experience.
• Current RN license in Illinois.
• CPR/BLS certification; CCM certification preferred.
