Job summary

St. Anthony North Hospital is seeking an RN Care Coordinator to manage care progression and discharge planning for identified patients, ensuring quality and cost-effectiveness while aligning with hospital policies.

Qualifications

• Requires RN license and acute hospital experience.
• Minimum two years of acute hospital clinical experience or a Master's in Case Management in lieu of 1 year.
• BLS certification within 3 months of hire.

Responsibilities

• Oversee progression of care and discharge planning for identified patients.
• Coordinate with utilization management, nursing, physicians, insurers, and post-acute providers to progress care.
• Advocate for patients and families, addressing barriers to care transitions.
• Develop and document a multidisciplinary discharge plan engaging the health team and postacute providers.
• Identify readmission risks and implement mitigation strategies for 30 days.
• Oversee and evaluate the discharge plan and resource utilization.
