VCU Health is seeking an RN Care Coordinator to optimize transitions of care from hospitalization through post-discharge. You will work with physicians, social workers, discharge planning, and payers to ensure safe, efficient, and cost-effective patient care.

Virginia-licensed RNs with at least 3 years in acute care are preferred, with academic healthcare experience considered an asset. Shifts include day and evening coverage to support patient needs.

Qualifikationen

• Virginia RN licensure required.
• Minimum of three (3) years of nursing experience in an acute care setting.
• Academic healthcare experience preferred.

Aufgaben

• Coordinate transition from acute care to post-discharge with physicians, social workers and payers.
• Collaborate with multidisciplinary team to ensure patient needs are met and care is coordinated across the continuum.
• Act as liaison between hospital and community agencies for referrals and information exchange.
• Ensure adherence to CMS guidelines and participate in patient safety initiatives.
