Qualifications

• Baccalaureate Degree in Nursing from an accredited School of Nursing is required.
• Current RN licensure in Virginia is required.
• Minimum of three (3) years of nursing experience in an acute care setting is required.
• One (1) year of UM/Care Coordination experience is preferred.
• Master’s Degree in Nursing or healthcare related field is preferred.

Responsibilities

• Collaborates with attending and consulting physicians to facilitate transitions through hospitalization.
• Works with the multi-disciplinary team to coordinate care across the continuum and assure payer reimbursement.
• Resolves psychosocial care issues with social workers and develops complex discharge plans as needed.
• Interacts with patients, families, and community resources to ensure needs are met.
• Ensures compliance with Quality Improvement Organization (QIO) and CMS guidelines.
