Job summary

VCU Health is seeking a Registered Nurse Care Coordinator to join a multidisciplinary team, guiding patients from the acute hospital stay through post-discharge to ensure high-quality, efficient, and safe care. The role emphasizes collaboration with physicians, social workers, discharge planners, and payers to coordinate transitions, manage resources, and address psychosocial needs.

RN licensure in Virginia and a BSN are required, with 3 years of acute care experience preferred.

Qualifications

• Minimum of three (3) years of nursing experience in an acute care setting.
• RN licensure in Virginia or eligible.
• Baccalaureate Degree in Nursing, or Bachelor's in biological/health or social sciences.
• Master’s Degree in Nursing or healthcare field preferred.

Responsibilities

• Works with attending and consulting physicians to facilitate effective transitions through hospitalization.
• Collaborates with multi-disciplinary team to coordinate care across continuum and ensure appropriate reimbursement.
• Seeks social work expertise to resolve psychosocial issues and develop transition/discharge plans.
• Maintains professional rapport with providers, patients/families, and internal customers.
