Job summary

Parkland Health and Hospital System in Dallas seeks a registered nurse to lead care coordination activities within the patient care team. You will assess, plan, and communicate care plans to patients, families, and staff, ensuring medical necessity and appropriate utilization.

The role requires nursing degree, Texas RN license (or compact), 2+ years hospital or community-based nursing, and BLS certification.

Qualifications

• Education: Must be a graduate of an accredited school of Nursing.
• Experience: Must have 2+ years of hospital or community based patient care nursing, preferably in assigned clinical area.
• Certification/Registration/Licensure: Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license.
• Must have current healthcare provider BLS for Healthcare Providers certification from one of the following: American Heart Association, American Red Cross, Military Training Network.

Responsibilities

• Conducts assessment of patients on assigned Care Coordination team to develop a case management plan of care.
• Collaborates with the multidisciplinary team to implement the plan of care and monitor patient progress.
• Communicates plan of care to patient and family and updates plans as needed.
• Develops reporting mechanisms to communicate outcomes to physicians and health care team.
• Responsible for Utilization Management activities for assigned patients, monitors length of stay and resource use.
• Ensures cost-effective care and optimal discharge planning with payer coordination.
