Job summary

Qualifications

• Must graduate from accredited school of nursing.
• Two years of nursing experience required.
• Texas RN license (or temporary) required; Compact license acceptable.
• Current BLS for Healthcare Providers from recognized organizations.

Responsibilities

• Conducts comprehensive patient assessment in person, by phone, or from records.
• Develops patient plan of care and communicates it to patients and families.
• Oversees implementation of care plan and ensures scheduling of appointments.
• Reviews referrals for appropriateness and coordinates clinical workups as needed.
• Acts as patient advocate and liaises between provider and patient/family.
• Maintains documentation and manages payer information for reimbursement.
