Qualifications

• Must be a graduate of an accredited school of Nursing.
• Must have completed two years of hospital or community-based patient care nursing.
• Must hold a current RN license or Texas Compact RN license; BLS for Healthcare Providers.
• Able to use PC and Windows-based software and understand Medicare/Medicaid rules.

Responsibilities

• Assess patients on the Care Coordination team to develop a plan of care.
• Coordinate with physicians and the health care team to implement the plan.
• Identify funding sources and eligibility for patients; plan and determine actions.
• Monitor patient progress and communicate updates to patients, families and staff.
