Job summary

Parkland Health and Hospital System (PHHS) is seeking a Registered Nurse for the Care Coordination team at New Parkland Hospital. The role is on-site with day shifts (7:30am-4:00pm) and PRN employment.

You will assess patients, develop care plans, and coordinate services across physicians, families, and Parkland staff to ensure quality care. The position requires a Texas RN license, BLS certification, and at least two years of hospital or major care experience.

Qualifications

• Must be a graduate of an accredited school of Nursing.
• Must have two (2) years of hospital or community based patient care nursing.
• Must have current, valid RN license in Texas or compact RN license and BLS for Healthcare Providers.

Responsibilities

• Assess patients on Care Coordination team to develop a plan of care and identify funding sources.
• Collaborate with multidisciplinary team to implement and monitor the plan of care.
• Communicate plans and progress to patients, families and Parkland staff.
• Utilize review criteria to ensure appropriate bed status and discharge planning.
• Support utilization management and identify opportunities for cost effective care.
