Job summary

Parkland Health in Dallas is seeking an experienced Registered Nurse to join our Care Coordination team on the Main Hospital Building, first floor. This full-time, onsite role operates days (8:00am-4:30pm), focusing on assessment, care plans, and utilization management to support quality patient care.

You will collaborate with physicians, families, and the care team to determine eligibility, coordinate discharge, monitor length of stay, and secure appropriate funding sources.

Qualifications

• Graduate of an accredited school of Nursing.
• Two years of hospital or community-based patient care nursing experience.
• Current RN license (Texas/compact) and BLS healthcare provider certification.
• Ability to communicate and collaborate with diverse patients, families, and staff.

Responsibilities

• Assess patients on Care Coordination team to develop a plan of care with input from patient, physicians and care team.
• Determine funding sources and eligibility for services.
• Monitor plan of care, LOS and resources; ensure patient needs are met and discharged appropriately.
• Collaborate with multidisciplinary team to implement and adjust the care plan.
• Coordinate transitions through the health care continuum and engage in performance improvement opportunities.
