Job summary

Duke Health Lake Norman Hospital in Mooresville, NC seeks a Registered Nurse Care Coordinator to coordinate patient care across inpatient, outpatient, and home health settings. You will serve as a clinical expert for complex patient populations and act as a liaison for the care team to optimize outcomes.

The role includes education for patients and families, care coordination/transition management, and collaboration with providers to ensure high-quality care.

Qualifications

• Knowledge of scope for a registered nurse.
• Knowledge of and appropriate application of the nursing process.
• Ability to independently seek out resources and work collaboratively.
• Ability to establish and maintain effective working relationships.
• Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others.
• Ability to teach patients and families in accordance with the nursing plan of care.

Responsibilities

• Collaborates with providers, case managers, registered nurses and other health care team members to assess patient needs and develop a plan of care.
• Provides triage support for patient populations across multiple clinical areas.
• Monitors patient outcomes and documents patient responses to care.
• Ensures providers are informed on outcomes.
• Serves as the point of contact for their patient population or care team to manage complex health needs.
• Provides patient consultations and information on recommended treatments.
• Coordinates prescription refill management and prior authorization forms.
• Completes clinical paperwork including lab, school, and leave forms, medical releases, emergency, and travel letters, durable medical equipment, etc.
• Assists in coordinating complex patient visits.
• Supports patients through pre-visit clinical assessments and history collection.
• Participates in QAPI activities.
• Educates patients and caregivers about services, disease states, and interventions to support health status.
