Duke Health Lake Norman Hospital in Mooresville, North Carolina is seeking a Registered Nurse Care Coordinator to join our care team. You will coordinate patient care across inpatient, outpatient, and home health settings and act as the clinical expert for complex patient populations, including transplant and heart services.

This role requires a BSN, 3 years of nursing experience, and current NC RN licensure.

Formación

• BSN required.
• Current NC RN licensure.
• 3 years of nursing experience.
• Ability to coordinate across care settings.

Responsabilidades

• Coordinate care across inpatient, outpatient and home health settings.
• Provide clinical guidance to care teams and act as a clinical expert for complex patient populations.
• Triages and communicates patient needs to the care team.
• Educate patients and families about treatments, medications, and follow-up care.
• Assist in discharge planning and transitions from hospital to home health.
• Document care plans, assessments and outcomes in the medical record.
• Support quality improvement and regulatory compliance activities.
