ComForCare in Oklahoma City is seeking a Registered Nurse to apply clinical knowledge without direct patient care. The RN will perform in-home visits focusing on assessment, conversation, and care planning for non-medical caregivers.

Each visit centers on understanding health history, functional needs, and personal goals to build realistic care plans. Travel to clients' homes and collaboration with the office team are required; no clinical procedures or lifting.

Qualificações

• Active RN license and current certification to practice as a nurse.
• Ability to perform thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.
• Professional, compassionate communication with clients and families.

Responsabilidades

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
