Responsibilities

• Develops, implements, coordinates, and evaluates individualized healthcare plans with interdisciplinary teams to improve access and reduce hospitalizations.
• Collaborates with providers and staff to identify appropriate patients for care management.
• Performs comprehensive assessments and prioritizes patients based on need.
• Educates patients and families on care choices and self-management goals.
• Maintains timely documentation in the EMR and care management databases.
• Manages a caseload of approximately 50–100 patients and meets productivity standards.
