The role is hybrid with full-time hours and involves collaboration with PCPs and care teams to close gaps in care and guide patients through transitions.

The ideal candidate has 4+ years of RN experience, strong communication, and proven ability to work autonomously while managing complex patient needs.

Qualifications

• Requires Wisconsin RN licensure (or MULTI-state NLC).
• Bachelor's degree in Nursing preferred.

Responsibilities

• Provide chronic care management for high-risk/complex patients.
• Collaborate with PCP and care teams to establish patient-centered goals and a care plan.
• Close gaps in chronic and preventive care; support transitions across care continuum.
