Job summary

MLK Community Healthcare seeks an RN Coordinator for Enhanced Care Management (ECM) in California. The role coordinates care for ECM enrollees, leads care plans, and collaborates with the interdisciplinary team to optimize patient outcomes.

This on-site position emphasizes nursing assessments, medication management, and transitions of care to the appropriate setting. The RN Coordinator will engage in social determinants of health, support home visits when needed, and ensure adherence to program

Qualifications

• Minimum of 1 year in case/care management preferred.
• Must be able to build effective relationships with peers and leaders.
• Experience coordinating care across disciplines is valued.

Responsibilities

• Provide clinical oversight of ECM Lead Care Managers and develop patient-centered care plans.
• Ensure ECM members receive required services and track progress against care plans.
• Coordinate care with PCPs, behavioral health providers, and specialists; support transitions in care.
• Conduct nursing assessments and address social determinants of health and housing needs.
• Assist with billing submissions and chart audits as needed.
