Qualifications

Responsibilities

• Lead discharge planning and care coordination for ED patients.
• Collaborate with providers, SWCC, CMA to ensure timely progression and discharge readiness.
• Document progress notes accurately in the electronic health record (EHR).
• Participate in interdisciplinary rounds and daily progression huddles to optimize throughput.
• Mentor and precept staff and new hires as needed.
