Job summary

VCU Health seeks an RN Care Coordinator to drive quality and efficiency across the hospitalization to post-discharge continuum. You will work with physicians, social workers, discharge planners, and payers to ensure safe, cost-effective care transitions.

The role requires Virginia RN licensure, 3+ years in acute care, and a BSN or related degree. Master’s in nursing is preferred, with strong collaboration and communication skills to support patient outcomes.

Qualifications

• Minimum 3 years of nursing experience in an acute care setting.
• Licensed RN in Virginia or eligible.
• BSN or related health sciences degree required.
• Current Virginia RN licensure required for employment.

Responsibilities

• Coordinate care with attending and consulting physicians to facilitate effective transition through hospitalization.
• Collaborate with the multidisciplinary team to ensure patient needs are met and care is coordinated across the continuum.
• Consult social workers to resolve psychosocial issues and develop discharge plans as needed.
• Interact with patients, families, healthcare professionals, and community/state agencies.
• Serve as an advanced clinical resource to the team.
• Liaise between hospital and community for exchange of clinical and referral information.
• Ensure compliance with CMS/QIO guidelines and use medical necessity criteria to assess care level and setting.
• Participate in patient safety initiatives and related responsibilities.
