Job summary

VCU Health is seeking an RN Care Coordinator to guide patients from the acute hospital setting through discharge, ensuring safe, efficient transitions and high-quality outcomes.

You will work with physicians, social workers, and payers to coordinate care across the continuum, optimize resource use, and maintain compliance with regulatory requirements. BSN is required; MSN preferred for advancement.

Qualifications

• RN licensure in Virginia or eligible
• Minimum of three (3) years of nursing experience in an acute care setting
• BSN required; MSN preferred

Responsibilities

• Coordinate transition through hospitalization with the attending and consulting physicians to facilitate effective and efficient discharge planning.
• Collaborate with the multidisciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, with appropriate reimbursement by payers.
• Engage with patients, families, and community providers; serve as a clinical resource and liaison between hospital and community agencies.
• Ensure hospital compliance with CMS guidelines, participate in patient safety initiatives, and assess and report potential risk management issues.
