VCU Health in Richmond, VA is seeking a Care Coordinator (RN) for the Neurology service to guide patient care from acute hospitalization through post-discharge. You will collaborate with physicians, social workers, and payers to ensure efficient transitions and high-quality outcomes.

Responsibilities include coordinating care across the care continuum, ensuring appropriate reimbursement, and maintaining compliance with CMS guidelines while supporting patient safety initiatives.

Formación

• Licensed Registered Nurse in Virginia or eligible.
• Minimum of three (3) years of nursing experience in an acute care setting.
• Bachelor of Science in Nursing from an accredited school, or a Bachelor’s in a biological/health sciences field from an accredited school.
• Master’s Degree in Nursing or a healthcare-related field from an accredited program (preferred).

Responsabilidades

• Works with attending and consulting physicians to facilitate transitions through hospitalization.
• Collaborates with multidisciplinary team to coordinate care across the continuum and ensure appropriate reimbursement by payers.
• Maintains compliance with CMS guidelines and QIO series; participates in patient safety initiatives.
• Establishes professional rapport with providers, patients/families, and internal customers.
