Job summary

UTMB Health in Webster, TX is seeking an RN Care Coordinator for the Care Management team. This casual, part-time role (up to 19.99 hours per week) partners with physicians, nurses, and ancillary services to plan, coordinate, and safely transition acute care patients to the next level of care.

The RN will assess discharge needs, develop transition plans, and lead multidisciplinary communication to ensure timely utilization of services and quality outcomes.

Qualifications

• RN with an Associate degree in nursing.
• Two years of clinical nursing experience in an acute hospital environment.
• Current license or valid permit to practice professional nursing in Texas.
• Certification in Care Management CCM or ACM-RN to be obtained within 3 years from the date of hire.

Responsibilities

• Coordinate discharge planning and transition to ambulatory settings.
• Collaborate with physicians, nurses, and ancillary services.
• Develop and implement safe, timely care transitions.
• Lead multidisciplinary care planning and performance improvement activities.
• Ensure effective communication and documentation.
