Job summary

MidState Medical Center is hiring a Care Coordinator (RN) to coordinate and monitor the plan of care for optimal discharge planning. You will work within an interdisciplinary team to expedite interventions and improve outcomes for patients.

The role requires a CT RN license with 3-5 years of clinical experience; BSN preferred and CCM certification desired. Strong communication, analytical and problem-solving skills are essential.

Qualifications

• Current CT RN License with 3-5 years clinical experience required.
• BSN strongly preferred.
• CCM certification is strongly preferred.

Responsibilities

• The Care Coordinator (RN) coordinates and monitors the plan of care to achieve optimal resource utilization and discharge planning outcomes.
• Expedites value added interventions to achieve desired outcomes and offers creditable alternatives to secure prompt and safe discharge.
• Serves as a change agent to improve care coordination, customer satisfaction, organizational and financial outcomes.
