Job summary

MidState Medical Center is seeking a Registered Nurse Care Coordinator to lead the coordination and monitoring of care plans, optimize resources, and support safe and timely discharges. Qualifications

• Current CT RN license with 3–5 years clinical experience.
• BSN strongly preferred.
• Experience with utilization management and managed care.
• Excellent written and verbal communication skills and patient assessment skills.
• CCM certification preferred
• Strong organizational and analytical skills
• Case management experience preferred
• Familiar with Medicare Part B
• Strong computer skills

Responsibilities

• As part of the interdisciplinary team coordinates and monitors the plan of care to achieve optimal resource utilization and discharge planning outcomes. Expedites value added interventions to secure prompt and safe discharge.
