Job summary

MidState Medical Center, part of Hartford HealthCare, is seeking a Care Coordinator (RN) to join the interdisciplinary team. You will coordinate and monitor the care plan to achieve optimal discharge planning and resource utilization, expediting value-added interventions and ensuring safe transitions for patients.

Requirements include CT RN license with 3–5 years of clinical experience; BSN strongly preferred; utilization management and managed care exposure; and strong written and verbal

Qualifications

• Current CT RN license with 3–5 years clinical experience.
• BSN strongly preferred.
• Experience in utilization management and managed care.
• Excellent written and verbal communication skills.
• Strong patient assessment skills.

Responsibilities

• Coordinate and monitor the plan of care to optimize resource use and discharge planning.
• Expedite value-added interventions to achieve outcomes.
• Offer creditable alternatives to secure prompt and safe discharge.
• Serve as change agent to improve care coordination, satisfaction, and outcomes.
