Job summary

MidState Medical Center is seeking an RN Care Coordinator to coordinate and monitor the plan of care with the interdisciplinary team, aiming for optimal resource utilization and discharge planning outcomes. The role expedites value-added interventions to secure prompt and safe discharge and acts as a change agent to improve care coordination and outcomes.

Candidates must hold a CT RN license with 3-5 years of clinical experience; BSN is strongly preferred.

Qualifications

• Current CT RN License with 3-5 years clinical experience required.
• BSN strongly preferred.
• Experience with utilization management and managed care preferred.
• Excellent written and verbal communication skills and patient assessment skills required.
