Job summary

MidState Medical Center in Connecticut is seeking a skilled Care Coordinator (RN) to join our interdisciplinary team. The role coordinates and monitors the plan of care to achieve optimal resource utilization and safe discharge.

Responsibilities include expediting value-added interventions, improving care coordination and patient satisfaction, and supporting organizational and financial outcomes. CT RN license and 3–5 years of clinical experience are required; BSN preferred, with utilization

Qualifications

• Current CT RN License with 3-5 years clinical experience required.
• Candidate with previous experience with utilization management and managed care.
• Excellent written and verbal communication skills and patient assessment skills required.

Responsibilities

• Coordinate and monitor the plan of care to achieve optimal resource utilization and discharge planning outcomes.
• Expedite value-added interventions to achieve desired outcomes and secure prompt and safe discharge.
• Serve as a change agent to improve care coordination, customer satisfaction, and financial outcomes.
