Qualifications

• Requires an RN license (single or multi-state), BLS certification, and at least 1 year of acute care nursing experience.
• Must possess excellent communication, assessment, organizational, and problem‑solving skills, with knowledge of healthcare regulations and community resources.
• Ability to work collaboratively in a fast‑paced environment under stress is essential.

Responsibilities

• Conduct psychosocial and functional assessments, collaborate with care teams to develop individualized discharge plans, facilitate interdisciplinary rounds, and manage post-acute referrals.
• Proactively identify and resolve barriers to discharge, ensuring timely and efficient patient transitions.
• Maintain accurate documentation of assessments, care plans, and interactions, while supporting department goals and professional development.
