Responsibilities

• Assess transitional care needs and coordinate across the continuum
• Collaborate with physicians, care teams, and social services to develop discharge plans
• Manage all aspects of discharge planning and address barriers to timely discharge
• Facilitate communication among care team members to promote efficient care progression
• Document assessments, care plans, and interactions in medical records
• Mentor and precept new staff and interns
• Support professional development and ensure compliance with policies and standards
