Qualifications

• Minimum of 1 year of RNCC or Case Manager experience.

Responsibilities

• Supports the progression of care for complex patients. Completes clinical assessments and participates in patient care rounds to ensure critical interventions and procedures.
• Coordinates transitions from hospital when medically indicated and participates in insurance authorization activities.
