Responsibilities

• Provides clinical coordination services including assessment, planning and intervention.
• Facilitates clinical transition planning for medically complex patients from hospital to home or next level of care.
• Collaborates with care teams to ensure timely discharge planning and appropriate bed/length of stay.
• Identifies barriers to care and escalates to leadership for resolution.
• Participates in post-discharge planning and community resource referrals.
