Job summary

Allina Health is seeking an RN Case Manager for a .6 FTE role (48 hours per two-week pay period) on a day shift with occasional weekend rotation. This position requires at least 1 year of RN Case Manager experience and a nursing license in MN or WI, with certification in Case/Care Management within 2 years of hire.

You will coordinate clinical transitions for medically complex patients, assess care needs, collaborate with the care team and families, and navigate preregistration and insurance

Qualifications

• Bachelor's degree in nursing is required.
• 2+ years of nursing experience.
• Case/Care Management Certification within 2 years of hire, with exceptions for Mother/Baby units or Cancer Institute requirements.

Responsibilities

• Coordinate clinical transition planning for medically complex patients from hospital.
• Assess and plan patient care and participate in rounds to ensure timely interventions.
• Identify barriers to care and escalate to leadership for resolution.
• Collaborate with health care team, patients and families for timely transitions and discharge planning.
• Support palliative and advanced care planning and ensure appropriate discharge planning and referrals.
