Allegheny Health Network is hiring a case management professional to develop, implement, and evaluate care plans across the health continuum. You will collaborate with providers, clinical teams, health plans, and external partners to optimize outcomes and resource use.

The role emphasizes readmissions reduction, length of stay management, ED utilization, and patient/family satisfaction, with ongoing education for patients and teams.

Qualifications

• Bachelor’s Degree in Nursing is required.
• 3 years in a clinical nursing role.
• Current State of PA RN licensure or multi-state licensure via eNLC.
• Demonstrated critical thinking and problem solving.
• Strong communication and collaboration with diverse teams.

Responsabilités

• Contributes to comprehensive assessment of patients and families/caregivers including social determinants of health and barriers.
• Identifies barriers and analyzes transitions and care coordination risks.
• Coordinates clinical care with patient, family, providers and interdisciplinary team.
• Advocates for patients with providers, payers, and post-acute partners to ensure outcomes.
• Educates patients, families, and teams on care appropriateness, documentation, utilization, and resources.
• Pursues professional development through certification and committee involvement.
• Adheres to hospital policies and regulatory requirements.
