The position requires a Bachelor of Science in Nursing, 2+ years of nursing experience, MN or WI RN license, and Case/Care Management Certification within 2 years. Day shifts with occasional weekend rotation and 8-hour shifts.

Qualifications

• Bachelor's degree in nursing required.
• 2+ years of nursing experience required.
• Licensed Registered Nurse - MN Board of Nursing required or Licensed Registered Nurse - WI Dept of Safety & Professional Services required.
• Case/Care Management Certification within 2 years of hire required (with exceptions as stated).
• Current BLS certification requirements for Cancer Institute positions or in-house BLS training within 30 days of hire.

Responsibilities

• Provides clinical coordination services including assessment, planning and intervention.
• Facilitates clinical transition planning for medically complex patients from the hospital when medically indicated.
• May provide initial and concurrent level of care review and insurance authorization activities.
• Supports progression of care for complex patients and ensures timely discharge planning and handoffs.
• Collaborates with healthcare team and community care providers to ensure effective transition of care and avoid readmissions.
